I understand that my child can only be released at the end of the day to one of the designated people on the registration form. My child may not leave the campus, without being checked out by a designated adult.
I agree to allow my child/ward to participate in Bethany Church’s Vacation Bible School (VBS) and affirm that my child’s/ward’s participation is completely voluntary. I understand that there are risks inherent in the activities my child will engage in at VBS which may cause serious injury. I also understand, that despite safety precautions, Bethany Church (including staff, volunteers, elders) cannot guarantee that my child/ward will not be injured. I agree to release, waive, hold harmless, discharge and agree not to sue the administrators of the VBS, or its trustees, officers, employers, volunteers and staff from any and all liability, claims, actions, demands, expenses, attorneys fees and medical expenses that I might now have or acquire in the future, arising out of or related to any loss, damage or injury that may be sustained to my child/ward from participating in VBS.
I give permission for photographs taken of me/child/ward while participating in VBS to be used for publicity purposes, which may include the newspaper.
